To request a Certificate of Insurance, email us at certs@clinewood.com. Please include the following information:
- The name of our customer (who our insured is).
- The name of the certificate holder.
- Certificate holder address.
- Email address of where the certificate needs to be sent (Preferred method of delivery) OR Fax number the certificate needs to be sent to.
- A call back number and contact name in case we need to reach them.
If during normal business hours [8am-5pm CST] you have not received your certificate within one hour please contact our office at once
